Medicare Rebate Information for Exercise Physiology Services

At Healthy Matters, we understand the financial burden that managing chronic health conditions can place on patients. That’s why we’re pleased to offer Medicare rebates for eligible patients receiving exercise physiology services.

Here’s what you need to know:

Eligibility for Medicare Rebates

Patients with chronic medical conditions and complex care needs, managed by their GP under a GP Management Plan (GPMP) and Team Care Arrangements (TCA), may be eligible for Medicare rebates for allied health services. Our exercise physiologists are registered to accept team-based care plans from GPs, providing eligible claimants with subsidised sessions. For the duration of your care, our allied health practitioners will provide your GP with regular reports that contribute to a comprehensive team care plan, ensuring continuity and quality of care.
Medicare rebate is available for up to five services per patient, per calendar year.

Understanding Chronic Medical Conditions

A chronic medical condition is defined as one that has been (or is likely to be) present for six months or longer. Common conditions include, but are not limited to:

- Asthma
- Cancer
- Cardiovascular disease
- Diabetes
- Musculoskeletal conditions
- Stroke

Chronic Disease Management (CDM) items are designed for patients who need an ongoing structured care approach from a multidisciplinary team, allowing GPs to effectively plan and coordinate the care of patients with complex conditions.

Eligible Patients

Community-based patients may be eligible if they have a chronic (or terminal) medical condition and their GP has provided the following CDM services:
GP Management Plan (GPMP) (Item 721)
Team Care Arrangements (TCA) (Item 723)


If you believe you may be eligible to claim this service, please speak to your GP. Upon establishing eligibility, your GP will send Healthy Matters an EPC referral, enabling you to use a Medicare rebate to subsidise the referred number of sessions.
